Even after you fill out this application online,
you must bring or mail a payment to Northside Baptist Church.

Scholarships available upon request.

Early registration for basketball is $55;
after November 1st, the cost is $60.

Early registration for cheerleading is $55;
after November 1st, the cost is $60.

Basketball shorts are included in the cost.

Please make checks payable to Northside Baptist Church.

Authorization and Release of Liability. You will be asked to physically sign
this at Evaluations.

Evaluations and Orientation
-
Everyone must attend one basketball
evaluation or cheerleading orientation

They will take place at Northside Baptist Gym
as follows between 5:00 pm and 7:00 pm:

3 Year Old through Kindergarten Boys & Girls:
Mon., Nov. 16
-
1st through 3rd Grade Boys & Girls: Tues.,
Nov. 17
-
4th through 6th Grade Boys & Girls: Thurs.,
Nov. 19
